    • 2 Good Mobility: No difficulty getting around enjoys walks and going outside

    • 1 Poor Mobility: Difficulty getting up, hard to get into the position to eliminate, short walks only

    • 0 Bare Min Mobility: Needs assistance, pain meds/anti-inflammatories don't help

  • SOCIAL FUNCTIONS

    • 2 Interacts normally with family and other pets (plays)

    • 1  Some interactions with family and other pets (sitting with family at rest)

    • 0 Hides in closet/under bed, or avoids family and other pets

  • MENTAL HEALTH

    • 2 Enjoys play activities, doing favorite things

    • 1 Decrease in doing favorite things

    • 0  No interest in favorite things

  • NUTRITION/Hydration

    • 2 Good Appetite & water intake

    • 1 Poor Appetite/ poor or increased water intake-hand feeding, needs enticing

    • 0 No Appetite/ requires subcutaneous fluids
